The Spring Street Exchange Social Determinants of Health (SDOH) Benchmark Assessment is designed to provide comparison to peer organizations and recommendations for next steps toward a sustainable approach to addressing SDOH. This custom version was created in partnership with the Association for Community Affiliated Plans, and registrants will be benchmarked against the 2023 cohort of ACAP plans.
